2018 APA Highlight: Self-Harm and Suicide Death Among Adolescents and Young Adults

Held during the 2018 American Psychiatric Association (APA) Annual Meeting, experts discussed the current state of treatment-resistant depression (TRD) in adolescents and suicidality from different perspectives, including suicide prediction and risk stratification, intensive psychotherapeutic approaches to TRD, pharmacologic approaches to TRD, and the current evidence base for the novel treatment ketamine in this population.

Highlighting one aspect, Dr. Mark Olfson presented the identified risk factors for repeated nonfatal self-harm and suicide death over the following year among adolescents and young adults with nonfatal self-harm. Most patients with nonfatal initial self-harm were white, adolescent, female subjects, and their events involved poisoning. Depression and substance use disorders were the most commonly diagnosed mental disorders within the 180 days preceding self-harm. The risk of repeated self-harm during the first follow-up year was over 100 times greater than the risk of suicide. Suicide risk after self-harm was ~4 times greater for male subjects than female subjects.

  • Sexual and gender minorities
  • Attempt history: Prior suicide attempt, especially violent means
  • Family Suicide History: Present
  • Psychiatric History: Inpatient admission(s), mood disorders, substance use, anxiety disorders
  • Developmental History: Sexual of physical abuse
  • Access to firearms
